Records Request 

PGFPD complies with C.R.S. Title 24, Article 72 Public Records (CORA)- Colorado Open Records Act and all applicable with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) Standards.

Basic Incident Reports are public information that may be requested by anyone. However, a Patient Care Report contains information that is legally protected by HIPAA. That information is called PHI (Protected Health Information). We are required by law to maintain the privacy of this information.

Requesting an Incident Report
If you are interested in obtaining a copy of an incident report (fire or medical), fire investigation report, hazmat report, or fire history report, please download and fill out the required information on our record’s request form.

You may also use an authorized law firm or public agency’s legal record request form. All record requests must be submitted one of the following ways:

  1. Email completed form to: records@pgfpd.org 
  2. Request completed form by mail to:

  3. Drop off completed request to:

Health Insurance Portability and Accountability Act of 1996 (HIPAA)

Patient Care Reports are PHI, and are not publicly available records. Access to this information is permitted only for the patient or their legally designated representatives. Among other things, the Health Insurance Portability and Accountability Act of 1996 (HIPAA) outlines the requirements for use and protection of individually identifiable health information, and the rights afforded to patients in regards to their information.

PGFPD adheres to strict Privacy Practices as outlined by HIPAA, and all personnel consistently practice, and train in the protection of your personal health information (PHI).
